(Psychiatric) Health Care: Top 5 Issues (and Their Pragmatic Solutions!)

Managing health care needs a helicopter view!

Kees Bakkenist and Hans Lodder have executed a limited preliminary survey under stakeholders in the (psychiatric) health care. The objective was to check whether there are important common issues. The findings show that in the near future organizations will have to deal with very important problems, strategic as well as operational.

Our survey was executed under executives, management and employees from our own network, built during our more than 20 years of extensive consultancy in the field of (psychiatric) health care.

We classified our findings in the order of:

  1. Easy to fix
  2. Highest ROI
  3. Results fast visible

It is interesting to notice that solving many of these issues have a positive effect on cash-flow and profit. Our findings:

  1. 15% of the patients do not show up at an appointment ("No shows")
  2. The number of security and privacy violations are sharply increasing
  3. A large number of patients suffers from cross organization medication issues
  4. A large number of business process rule violations causes image damage and severe losses
  5. Simple and straightforward standardization of the business processes cause severe issues with patients and employees

15% of the patients do not show up at an appointment ("No shows")

For certain groups of patients it is difficult to be disciplined enough to keep made appointments at a polyclinic. This does not have to be on purpose, but they just might forget to go there. The consequences are a less effective treatment of the patient, and an expensive business process: It is not efficient if everything is ready for the patient, but he just does not turn up.

Many organizations solve the issue easily by having the patient called up by an automatic program to remind him of the appointment. The ROI of this solution is very high.

The number of security and privacy violations are sharply increasing

Security and privacy are of the utmost importance for health care. Yet most of the organizations have no validated indication of the number of daily violations. The consequence of this is that one time or another the organization will suffer from severe image damage, for instance when the dossier of an important person becomes widely available.

Many organizations solve this issue by installing a specialized system that meticulously follows all traffic, and checks more than 100 protocols. The implementation of this system tends to help you rethinking your business processes, and has as a secondary benefit a sharp increase of the effectiveness of your business processes.

A large number of patients suffers from cross organization medication issues

It is estimated that in the Netherlands alone yearly 2,000 patients are killed by wrong medication. Medication is supposed to be part of the EPD in order to enable tracking and tracing of medicine prescriptions. Especially when medical staff of different organizations (or parts) are dealing with the same patient, problems can arise.

Many organizations implement a specific solution to enforce this tracking and tracing of medication. That solves the problem adequately.

A large number of business process rule violations causes image damage and severe losses

The business processes in (psychiatric) health care are very complex, and they are subject to very frequent changes. The consequences of this are that it is very difficult to retain a helicopter view on all business processes. Its is unavoidable that business process rule violations are very frequent. This easily results in possible image damage, or at lest severe efficiency penalties.

Many organizations implement a specific solution to enforce all high level business rules, especially the cross organization ones. A 2nd benefit is the implementation tends to streamline your business processes, which has a strong positive effect on your cash-flow.

Simple and straightforward standardization of the business processes cause severe issues with patients and employees

In the Netherlands it seems to be fashionable to make health care processes Lean and / or 6Sigma compliant. In reality health care processes vary strongly, and with good reasons. The 6Sigma approach tends to result in a sub-optimization. It takes the heart out of the health care system, and dehumanizes specific patient care. The consequence is de-motivated patients and de-motivated personnel, and therefore expensive operational processes.

Many organizations use a more sophisticated approach, where all business requirements are balanced against each other. This multidisciplinary approach, using a helicopter view, allows for an effective cross business process optimization, and increases the motivation of patients and personnel. The operational efficiency is very positively affected. That solves the problem adequately.
